NOTICE OF DATA PRIVACY INCIDENT - Visiting Nurse Association of Texas ("VNA") is providing notice of a recent data privacy incident that may have affected limited protected health information (PHI) for certain individuals. On July 17, 2025, VNA experienced unusual activity on its network and immediately began a thorough investigation, which included working with third-party computer experts. The investigation determined that an unauthorized individual accessed certain information in employee email accounts. In order to determine the type of information present and to whom that information related, a comprehensive and time-consuming review of the contents of the potentially impacted email accounts immediately followed. In response to this incident, VNA also took steps to secure their environment and implement additional technical safeguards to further enhance the security of information in their possession and prevent future incidents. - In an abundance of caution, VNA is offering potentially affected individuals access to credit monitoring and identity protection services. To obtain more information about this incident or enroll in these services, individuals should contact VNA's dedicated assistance line at 1-844-784-5324 from 7:00am to 7:00pm Central time, Monday through Friday, excluding major U.S. holidays. As a best practice, it is always prudent to review health care statements for accuracy and report any services or charges that were not legitimately incurred to the provider or insurance carrier. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045. - Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and steps to protect their personal information by contacting the credit reporting bureaus, the Federal Trade Commission ("FTC"), or their state Attorney General. Instances of known or suspected identity theft should also be reported to law enforcement, your state attorney general, and the FTC.
